Position
The Quality and Risk Manager is responsible for leading the facility’s quality improvement, risk management, regulatory compliance, performance measurement, and continuous quality improvement activities. The incumbent works collaboratively with leadership, interdisciplinary teams, residents, families, and external stakeholders to promote a culture of safety, quality care, regulatory compliance, and organizational excellence. The Quality and Risk Manager supports the development, implementation, monitoring, and evaluation of programs designed to improve resident outcomes and ensure compliance with the Fixing Long-Term Care Act, 2021, Ministry of Long-Term Care requirements, and Accreditation Canada standards.
Quality Improvement Leadership
- Lead the home's Quality Improvement Program and annual Quality Improvement Plan (QIP).
- Monitor and analyze quality indicators, resident outcomes, clinical trends, and performance metrics.
- Adherence to compliance with Inspections conducted by Ministry of Long Term Care
- Knowledge and experience of quality indicators used and understanding of indicators that are publicly reported.
- Coordinate quality improvement initiatives using evidence-based methodologies.
- Facilitate continuous quality improvement projects and monitor outcomes.
- Prepare quality reports for leadership, committees, and governing bodies.
- Promote a culture of quality, accountability, and person-centered care.
- Provide frontline presence on the units to support, evaluate and better understand quality and barriers to quality.
Risk Management
- Develop, implement, and maintain the home's risk management program.
- Lead investigations of incidents, complaints, critical incidents, and adverse events.
- Conduct root cause analyses and develop corrective action plans.
- Monitor organizational risks and recommend mitigation strategies.
- Maintain systems for incident reporting, tracking, trending, and follow-up.
- Support emergency preparedness and business continuity planning.
- Regulatory Compliance
- Monitor compliance with applicable legislation, regulations, ministry directives, and organizational policies.
- Coordinate audit activities and inspection readiness initiatives.
- Support Ministry inspections and accreditation reviews.
- Track corrective actions arising from inspections, audits, complaints, and investigations.
- Participate in policy development and review to ensure legislative compliance.
Resident Safety and Clinical Quality
- Collaborate with clinical leaders to improve resident safety and quality outcomes.
- Monitor indicators related to falls, restraints, skin and wound care, medication safety, infection prevention and control, responsive behaviours, and resident satisfaction.
- Support implementation of best practices and evidence-based care.
- Identify emerging risks and quality concerns affecting residents.
- Data Management and Reporting
- Collect, analyze, interpret, and present data to support decision-making.
- Develop dashboards and performance reports.
- Monitor quality indicators and benchmark performance against internal and external standards.
- Provide recommendations for quality and operational improvements based on data analysis.
Education and Staff Development
- Develop and deliver education related to quality improvement, risk management, resident safety, and regulatory requirements.
- Coach leaders and staff on quality improvement methodologies and risk reduction strategies.
- Support orientation and ongoing staff education programs.
- Accreditation and Continuous Improvement
- Coordinate Accreditation Canada activities and readiness processes.
- Lead self-assessments, action planning, and continuous monitoring of standards.
- Support implementation of leading practices and organizational excellence initiatives.
Qualifications
- Bachelor’s degree in healthcare, health sciences, quality Improvement or a related field.
- Master's degree in a related discipline is considered an asset.
- Current registration in good standing with a recognized professional regulatory body in Ontario preferred.
Experience
- Minimum five (5) years of progressive experience in healthcare – preferably long-term care
- Minimum three (3) years of experience in quality improvement, risk management, regulatory compliance and safety.
- Experience preparing for accreditation surveys, audits, and ministry inspections considered an asset.
Certifications (Preferred)
- InterRAI LTCF certification and knowledge are considered strong assets.
- Lean Six Sigma Green Belt or Black Belt.
- Quality Improvement certification.
